a company), a letter or fax signed by each individual, or

stating the following: “I hereby give full permission
to Caroline Moroz to carry out an astrological analysis of
the relationship between myself [insert name #1] and [insert
name #2] and confirm that she does so in good faith that
both of the abovementioned parties are agreeable to the same”. |
